The Care Plan tab
Care Plan holds everything you prescribe or assign to a family, as opposed to Trends, which holds what they send back. Like Trends, it uses a rail: pick a section on the left and it opens beside it.
What is in the rail
Not every patient shows every section. The rail is built from the programs that family is on and the records their chart has turned on.

If a section you expect is missing, the cause is usually one of two things: the family is not on the program that supplies it, which you can change in Program Access, or the record is switched off for this chart, which you can change in chart setup.
Assigned to family
Beside the plan sits the Assigned to family board, which shows everything queued or live for that family in one place: surveys and screeners, routines, handouts, and exposure goals.
Use it to answer the question the plan itself cannot, which is what the family currently has in front of them rather than what you intended them to have.
Where the work comes from
Most of what lands here is created somewhere else and appears on this tab afterwards.
- Goals, meal plans, and nutrition targets are usually set during a session, in steps 3 and 4 of the session workflow.
- Handouts, screeners, and routines are assigned from the Library.
- Vaccine doses can be recorded directly on the Vaccines section, or by the family in their app.
Care Plan is where you review and adjust all of it between visits.
Setting a nutrition goal
A nutrition goal is the set of targets a patient's intake is measured against. Until you set one, the platform uses calculated USDA values; once you set one, your numbers govern.
Where to set it
Two routes reach the same goal:
- From Trends → Nutrition, click Set nutrition goal, or Update nutrition goal if one already exists.
- Inside a session, at Step 4, Nutrition Guidance, where setting targets is part of the visit.
Use the Trends route when you are adjusting between visits, and the session route when the change belongs to a visit you are documenting.
What the dialog does
It recalculates the USDA targets from the child's current measurements, then lets you adjust any nutrient by hand. Each row shows the calculated target, your adjusted target, and the patient's intake as a percentage of it.
Two checkboxes sit on every nutrient row and do different jobs:

Those are independent. A nutrient you are watching closely is not automatically something the family sees, which matters when tracking something you do not want a parent anxiously counting.
Saving
Saving records a new goal and leaves previous prescriptions untouched, so the history of what you prescribed stays intact rather than being overwritten.
