Step 3 — Goal Setting
Patient-level goals that carry across sessions. Goals save the moment you add them (this step has no separate save), and active goals print on the Session Summary PDF and appear in the family's app when connected.

Managing goals
- Goals are attached to the patient, not the session; every session's Step 3 shows the same list.
- Statuses: active, completed, discontinued. Completed and discontinued goals collapse into their own section.
- Edit or delete anytime.

Step 4 — Nutrition Guidance
Three cards, one workflow: confirm the patient's metrics, set nutrition targets, build the meal plan.
Key Metrics
- Weight, Height, and Activity level (units follow your App preferences)
- Date of birth and age in days, with a Corrected age switch for preterm patients
- Last measured values, change, and percentile per metric
- Calculate USDA RDA computes the targets for the next card
Nutrition Planning
Targets show as USDA Target and Adjusted Target columns, with clinician levers:
- Adjust targets: override the calculated value for any nutrient
- Priority nutrients: flag the nutrients you want highlighted
- Show nutrition priorities to patient: one switch that makes your priority list visible in the family's app
Meal Planning
A generator plus an editable plan.
- Plan type: Magic (AI-optimized) · Easy and quick · Get more protein · More veggies and fruits · Introduce allergens · Introduce new foods · Family meals · Use few ingredients · Baby-led weaning
- Schedule: 1–30 days, with meal-type chips; options for repeat menus and family meals
- Constraints: allergens to avoid (pre-filled from the record), diet, foods to avoid; an infant-feeding block appears for patients under age 2
- After generating: per-day tabs, macro breakdown vs target, swap in recipes ("Add a recipe" searches the public recipe library) or generic food items, drag to reorder, Download PDF
Where the plan lands
The plan saves with the session and reaches the family when you sign the session in the Summary step: "The meal plan and nutrition goals appear in their mobile app." On your side it lives under the patient's Care Plan → Meal Plans. Families without the Heartful Sprout app get it as the PDF you download and send.
