Billing Settings
Your billing details — pulled into the Superbill and CMS-1500 when you generate them. Fill these in before your first billing document.
Where to set this
Profile → Settings → Billing Settings.
Fields

| Tax ID (EIN or SSN) | CMS-1500 |
| Billing phone | Superbill, CMS-1500 |
| Billing address (street, city, state, ZIP) | Superbill, CMS-1500 |
If these fields are empty when you generate a billing document, those areas of the form will be blank.
