Documenting a food reaction

For patients who report a reaction at home, use the structured path so it's usable for clinical decisions.

Patient logs it first (preferred)

  • Heartful Sprout: Health → Reaction Notes — date, symptoms (from a picker), and a free-text note.
  • Heartful Baby: meal logs capture liked/neutral/disliked only; for an actual adverse reaction, have the family log it in Heartful Sprout or tell you directly.

Logged reactions surface in your record under Trends → Nutrition, on the Food Reaction Notes card.

You log it in a session

If they call before the next visit and don't log it themselves, capture it in the session:

  1. Session Capture: note timing, symptoms, suspect foods.
  2. Session Notes: make the reaction explicit in the assessment.
  3. Goal Setting: adjust introduction goals if needed (e.g., pause specific allergens).
  4. Billing Codes: appropriate ICD-10 for the reaction (food allergy, dermatitis, etc.).

After

  • The family sees your patient note in their app once the session is signed.
  • If severe: prompt the family to follow up with their pediatrician and/or allergist.
  • Track future intros via Food intro.
