The most common return visit. Anchored on the prior session's goals and what the family logged since.

During the session

  1. Session Capture: usually shorter; dictate as you discuss progress.
  2. Session Notes: generate; emphasize Monitoring/Evaluation (ADIME) or Plan (SOAP) for goal progress.
  3. Goal Setting: refine, close, or add goals.
  4. Nutrition Guidance: minor adjustments to the existing plan often beat regenerating.
  5. Billing Codes: re-assessment is 97803, billed in 15-minute units (a 30-minute follow-up = 2 units).
  6. Summary: review, then Sign & finalize.
